MEDICALS REQUIRED - ALL MUST HAVE BEEN DONE WITHIN SIX MONTHS IF YOU’RE NOT ALREADY LICENSED.

  • Bloodwork: Hepatitis B Surface Antigen, Hepatitis C Antibody, HIV. You must have these specific tests done. We will set this up for you - we need your zip code and birthday. If someone has reached out to you for this information, please provide it so we can setup your bloodwork.

  • Physical: Must be done on the Illinois form, and MUST be done by an MD or a DO. A physical done by a nurse practicioner (FNP) or physician’s assistant (PA) WILL NOT BE ACCEPTED. Click here to download the form.We recommend Concentra, as they typically have an MD or DO in the office.
