We are excited to have you fighting with us on February 20, 2026. Here is some information, along with medical forms.

MEDICALS REQUIRED - ALL MUST HAVE BEEN DONE WITHIN SIX MONTHS IF YOU’RE NOT ALREADY LICENSED.

• Bloodwork: Hepatitis B Surface Antigen, Hepatitis C Antibody, HIV. You must have these specific tests done. We recommend using Request a Test, and ordering the Combative Trio, which includes the three required tests for $89.

• Physical: Must be done on the Illinois form, and MUST be done by an MD or a DO. A physical done by a nurse practicioner (FNP) or physician’s assistant (PA) WILL NOT BE ACCEPTED. Click here to download the form.

• Eye Exam: Must be done on the Illinois form, and MUST be done by an OPTHALMOLOGIST. An eye exam done by an optometrist WILL NOT BE ACCEPTED. Click here to download the form.
